YASSER ALBA CARLOMAN I, _______________________________________, respectfully apply for a Certificate of Public Convenience ("C") to operate a Transportation Network Vehicle Service ("TNVS") under the Land Transportation Franchising and Regulatory Board ("LTFRB") Memorandum Circular (MC) No. 2015-017 (the "Application"). 1.

I am currently completing all the documentary requirements for the Application.

2. In the meantime, pursuant to Paragraph C of MC No. 2015-017, I respectfully request for Provisionally Authority to operate as a TNVS. I hereby name, constitute and appoint Uber Systems, Inc. and/or any of its employees, and the law firm of Quisumbing Torres and/or any of its lawyers, with office address at 12th Floor, Net One Center, 26th Street corner 3rd Avenue, Crescent Park West, Bonifacio Global City, Taguig City, Philippines, to act for and on my behalf in connection with, and to take all actions appropriate or necessary for the purpose of, pursuing this application for Provisional Authority in my favor.
