Terbutaline in Ascoril Expectorant causes tremors in patients ?  Doctor, I understand your concern about tremors with Terbutaline

 Doctor, Fine muscular tremors are very common & are less serious side effects seen with Terbutaline & Salbutamol (albuterol)  Both Terbutaline & Salbutamol belong to short acting Bronchodilator (Beta 2 agonist )

 Adverse effects, like tremors & tachycardia may occur more frequently with terbutaline. Hence, proper monitoring is needed. Tolerance to this effect can be expected within 1-2 weeks or chronic therapy  Adverse CNS or musculoskeletal effects (nervousness, headache, hand tremor) with albuterol (41%) terbutaline (73%), While with I.V. albuterol had a higher incidence of palpitations (14/20) than either terbutaline (7/20). Your Ascoril expectorant is a costly preparation as compared to other Salbutamol brands ?

 Doctor, you are well informed that Salbutamol brands are priced less compared to Terbutaline brands. This is mainly because of the pricing policy. But doctor carefully notice that these brands have either a combination of Salbutamol with Bromhexine/Ambroxol or Salbutamol with Guaifenesin. This means these brands either miss the vital Mucolytic/Mucokinetic or an Expectorant. That means these brands compromise on the efficacy i.e. Complete expectoration. Therefore, do you think without Bromhexine or Guaifenesin can there be a complete expectoration. This concentration is well tolerated and does not affect the taste of Ascoril Expectorant.  As Bromhexine is given along with Guaifenesin it works by drawing water into the bronchi. The water both thins mucus & lubricates the airway, facilitating removal of mucus by coughing. Hence in combination gives better results.

Terbutaline is not preferred in pediatric age group ?

Doctor ,as per text books, Terbutaline is recommended in children above 6yrs.,while Albuterol is recommended in children above 2yrs. But studies show that with Terbutaline oral dose of 0.75 milligram to 1.5 milligrams twice or three times daily has been recommended for children 3 to 7 years of age. For children 7 to 15 years of age, an oral dose of 1.5 to 3 milligrams twice or three times daily has been suggested (Brogden et al, 1973a). The dosage of terbutaline should be based upon body weight (Leegaard & Fjulsrug, 1973). A dose of 0.075 milligram/kilogram resulted in marked bronchodilation in asthmatic children.
