Manager / Supervisor undertake investigation consulting with relevant personnel. • Identify all root causes that contributed to event. • Assess risk and identify appropriate short, medium and long term corrective actions as required. • Record findings of investigation in Section C of incident / hazard report form and send copy to OHS • Notify management of report as per management unit internal procedure/s. • If corrective actions require FSG action - log work order request through WSM Enterprise
The manager with assistance of security if assistance required, ensures site of the incident is not disturbed until further advice received from WorkSafe.
OHS Consultant conduct investigation in conjunction with Work Safe Inspector, relevant manager / supervisor, and health & safety representative (where requested). Direction given by inspector and / or corrective actions to be implemented agreed to by all parties during investigation.
