ission Procedures •The ission Department is the patient's first stop when they enter the Medical Center. • Patients will be requested to present their Doctor's ission Order Sheet.
ission Procedures • In the absence of the Doctor's ission Order Sheet or a doctor known to the patient , a walk-in patient may be itted through the Emergency Room. •Patient fill out a Patient Information Sheet. Information requested will be needed by the attending physician and the Medical Center.
ission Procedures • Patient will indicate the accommodation of choice (suite, private room, semi-private - 2 patients per room, or ward - 4 to 6 patients per room). •Sign consent forms for hospital care, exclusive supply of medicines and limitation on outside diagnostic reports, release of information to insurance companies and/or patient's employer, and waiver of responsibility on loss of valuables.
ission Procedures • While ission is being processed and room is being prepared, patient will be asked to wait in the ission Waiting Lounge. As soon as the room is ready, patients will be escorted to the Nursing Unit by the ission Aide. All patients are given an ID band to be worn around the wrist for the duration of their stay at the Medical Center.
Reservation Procedures •The ission Department accepts room reservations. However, if the patient fails to arrive within the indicated time of arrival, the room reservation shall be assigned to other patients. The hospital also reserves the right to cancel reservations to give way to emergency and critical cases.
PRE- ISSION PHASE Receives patient from 1.ission Office ( DIRECT ), 2. Emergency Department ( From ER ), 3. General Units, Intensives ( Trans – in ) Receives telephone endorsement
Prepares room / bed for ission
Emergency issions •Patients are referred to the Emergency Room (ER) if they do not have an ission order sheet from their attending physician. In such cases, the ER consultant will ask the patient for the name of physician. The ER consultant may only assign an attending physician to your case if you do not have a personal physician of choice.
