HAAD Pre-Licensing Procedures (for SELF-APPLICANTS) Pasensya na po, I just made this after completing the process kaya may konting difference lang sa mga screenshots na nakalagay po dito… :p

After completion of Dataflow, you will receive an E-mail with your name and . There will be a link in the email that will re-direct to your HAAD . After Entering your name and , dito po kayo mapupunta….

 

Click “e-Licensing” Click “Health Professionals Licensing”

 

Click the button under “Select” on the lower left portion. Click your AGN no., yung tinuturo ng arrow

(Don’t mind yung Application Status, Completed by, at Date Modified… Lalabas yan pag nasimulan nyo na iprocess poi to.”

PERSONAL TAB  Next, fill up all the necessary details.  Sa photo, attach in JPEG format.  In Arabic name, be sure to enter your CORRECT Arabic Name because nagiging ground din po yan for Disapproval ng application.  Basically, you will see two buttons sa pinakababa (wala pa yang “Comment” na nakalagay), the “SAVE AS DRAFT” button and “SUBMIT” button.  After filling up all necessary info and ing your pic, click SAVE AS DRAFT.  NEVER CLICK THE SUBMIT BUTTON IF HINDI PA KUMPLETO YUNG MGA S NYO UNTIL THE LAST TAB!!!

EDUCATION TAB  Under this tab, you will need to click “Attachment”  Attach your DIPLOMA, TOR, and RLE in PDF format (I don’t know if pupwede po ang JPEG, PDF po kasi ako in ko nun and it went well naman…)  Click SAVE AS DRAFT after ing

LICENSE TAB  Click “Attachment”  your PRC LICENSE CARD (front and back) and updated CERTIFICATE OF GOOD STANDING (must not be more than 3months from the date of release, much better kung bago) in PDF format.  For the first 3 questions, tick “No”  For the 4th question, tick “Yes” and enter the issue date of your CGS (Cert of Good Standing)  Click SAVE AS DRAFT after filling up and ing

EXPERIENCE TAB  Click “Attachment”  your EMPLOYMENT CERTIFICATE/S in PDF format (kung ano po yung in and na nung Dataflow, yun din lang po ang mga iu dito.)  Click SAVE AS DRAFT after ing EXAMINATION TAB  Leave it out po if first time mo pa lang magtatake.  For second timers na po, please ask assistance. Hindi ko po alam… Peace! :p CME TAB  Leave it blank LOGBOOK TAB  Leave it blank.

ATTACHMENTS TAB Upon entering this Tab, you will see that may mga nakaattach ng documents sa ilang Fields dito. Dataflow already ed your documents na na- nila in the specific field kung saan dapat iu. First thing to do is to check isa-isa kung tama and complete yung mga na- na documents. Kung ano po yung lacking, yun na lang ang i especially yung BLS and updated na Certificate of Good Standing (within 3 months).

DOCUMENTS TO (for SELF-APPLICANTS) Country Specific High School copletion certificate(as per the PQR).  HIGH SCHOOL DIPLOMA Professional Qualification-Certificate/Diploma/Degree/Post Graduate  HIGH SCHOOL DIPLOMA  COLLEGE DIPLOMA  TRANSCRIPT OF RECORDS  RLE  BLS CERTIFICATE Transcript of Program of Study-include theory and clinical hours if applicable  TRANSCRIPT OF RECORDS Practicing Professional License - valid and current from home country or from the country where the relevant qualification was obtained if the applicants worked there  PRC LICENSE ID (front and back)  VALID CERTIFICATE OF GOOD STANDING  BOARD CERTIFICATE  BOARD RATING Relevent Experience to the position  EMPLOYMENT CERTIFICATES A certificate of Good Standing from your current place of work from the Regulatory Authority not more than 3 months old. This must be sent directly to the Health Authority Abu Dhabi  VALID CERTIFICATE OF GOOD STANDING Dataflow Verification Report  *ALREADY ATTACHED BY DATAFLOW Security Clearance Form signed by the applicant  SECURITY CLEARANCE FORM / CID (can be ed on this Tab) *, fill-up, scan, Full port copy(valid and current) and Visa(Residence or Visit) if available  PORT (first and the last page)

Photograph with background(Recent not more than 6 months)  SELFIE (joke lang)  2x2 (same as dun sa mga naunang in ma photo) Declaration Form by the Applicant  DECLARATION FORM BY THE APPLICANT (*, sign, scan, ) Declaration Form by the Sponsoring Facility  a blank DECLARATION FORM if SELF-APPLICANT Authorisation File  *ALREADY ATTACHED BY DATAFLOW INDIA-Mark sheet for the final year (all year mark sheets for applicants) Attachments(0) Afghanistan, Egyptian & Pakistan-Copy of the backside on the degree certificate ( for applicants having Afghanistan, Egyptian & Pakistani degrees/certificates) Attachments(0) Certificate of Authenticity and Verification (CAV) for applicants who have studied in Philippines  CAV OF SCHOOL CREDENTIALS Current and up to date detailed resume (In English)  UPDATED AND DETAILED RESUME in PDF FORMAT

   

After ing all the documents, click SAVE AS DRAFT Review all from Personal Tab to Attachments Tab. If you are sure that all required documents are ed and all required information are provided, pray and click SUBMIT Next step is to review and wait for your Approval to take exam.

*You can check progress of your application in BPM History tab!
