Milson Road Health Centre A consultation on relocating clinical services

1. Introduction – what are the proposals? NHS Hammersmith and Fulham is currently developing plans for the relocation of services from Milson Road Health Centre (1–13 Milson Road, W14 0LJ), and the subsequent sale of the site. The Diabetes Services was reviewed in 2010 with considerable involvement and engagement from patients and carers. The redesign of the Diabetes Services across the borough means the bulk of services currently provided at the centre are to be re-provided from three centres colocated with other diabetes services and will offer better access and quality for all patients across the borough. NHS Hammersmith and Fulham believes that with the new diabetes services, alongside wider plans to improve the quality and accessibility of health buildings around the borough, the Milson Road Health Centre is no longer needed as a site for community health services. We would like to relocate the remaining services to better sites offering easier access to the widest possible number of patients using the services. This consultation is open from week beginning 6th June 2011 to 02 September 2011 and seeks views on the proposals to relocate a range of services from Milson Road to sites around the borough. Current services provided at Milson Road Health Centre The intermediate diabetes community service s for a significant part of services currently provided at the centre. In order of numbers of patients the other services currently provided at Milson Road Health Centre are: Podiatry, Leg Ulcer Clinics, Family planning and well woman services, Child Health and Child Development clinics, Midwifery and Paediatric Audiology. The future structure and locations of diabetes services have been consulted on last year and therefore are not part of this consultation process. If you require further information about your diabetes services you should speak to your diabetes clinician in the first instance. Podiatry Patients are seen primarily at health centres, GP practices, hospital based clinics, day centres and some drop-in centres, or where necessary in a patient's home. Transport can be provided to some of the clinics for semi housebound patients. There is also limited domiciliary care offered to housebound patients. We recognise that a few patients will need transport to attend and receive this service at other sites such as White City Health Centre, Richford Gate, or Parsons Green Health Centre. To patients, each patient will be given an information leaflet for their personal arrangements and there will be someone to talk to about their appointments during the transition period. We will housebound patients who are unable to use public transport and ensure they are ed to attend their appointments. Leg Ulcer Clinics Currently Leg Ulcer Clinics are provided at White City Health Centre and Parsons Green Health Centre for Health. To improve access to this service we are looking to provide some leg ulcer clinics at GP Practices. Staff within The Leg Ulcer Service will speak to all the patients attending the service personally about the change of location and this will be followed by a letter.

Family planning and well woman service This service provides a free and confidential range of contraceptive and sexual health services including emergency contraception, contraceptive pills, coil and implant insertion, screening for sexually transmitted infections and cervical smears. There is an advice and information telephone line and patients can be seen without referral and GPs can also refer for a same-day service. There are a number of access points to this service around the borough with one clinic held at Charing Cross Hospital and three clinics at White City Health Centre per week. There are also clinics at the Parson’s Green Health Centre in the south of the borough. We are currently looking to relocate this service from Milson Road to Richford Gate Medical Centre which will maintain a service within the centre of the borough and improve public transport links for people living in this area. The service will also be able to work more closely with the GP Practice based at that site. Child health clinics The health visiting service adheres to the Healthy Child Programme and reviews children’s health, growth and development at specific stages. Children are seen in the baby clinic at Milson Road on average 1-2 weekly up to 12 weeks and then 3-4 weekly after that in the first year. Other baby clinics are currently offered at several GP Practices within this area. The current Child Development and Child Health Clinics provided at Milson Road are proposed to move to the Masbro Centre located next door. Midwifery This clinic currently runs on a Monday morning with up to 10 women attending each clinic. Three other clinics run on a regular basis at Brook Green Medical Centre (Tuesday afternoons), Cathnor Park Children’s Centre (Thursday afternoons) and at the White City Centre for Health (Friday mornings). It is proposed that the clinic at Milson Road will move to Richford Gate as this will be more central to a greater number of patients who have limited access. The service will inform patients of change of location by phone and letter. Paediatric Audiology Currently this service is also provided at Parson Green Health Centre. We are exploring the current facilities at Woodfield Road, W2 as this will give patients living in the north of the borough options for access until the new service starts at Hammersmith Hospital. Consultation timetable The formal 12-week consultation runs from 13th June 2011 to 2nd September 2011. During the consultation the proposals will be presented to Local Involvement Networks (LINks) and the Overview and Scrutiny Committee (OSC) for comment. We will prepare and distribute a consultation report within two weeks of the close of the consultation, which will include: what you told us and what we did, timetable for implementation, next steps.
